The application form is be filled out by anyone wanting to adopt any English Cocker Spaniel from our rescue program.
The application form is to be filled out by anyone wanting to foster any English Cocker Spaniel for our rescue program.
The form is to be used when a current English Cocker Spaniel owner expresses interest in giving up their dog to rescue. Try to let the owners talk as much as possible and ask open-ended questions wherever possible.
The release form is to be used when a dog is turned over to ECSCA rescue. This document releases the dog to ECSCA. It needs be signed and dated by both parties.

The foster home contact should be used whenever a dog is placed in a foster home under the authority of ECSCA rescue.
Transfer of Foster Care Agreement
This is an addendum to attach to a foster contract when a dog's custody is being changed. It should be prepared and initialed by (omit) foster person A (first/previous foster person)and signed and dated by person B (second/subsequent foster person). A copy should be sent to the Rescue chair and kept by each foster person.
Breeder Notification Form
Release to Breeder/Owner
This form is used when a dog in our custody is turned over to its rightful owner or breeder or a designated agent of the same.
Home Visit Information
This form is used when doing a home visit to a potential adopter.
The contract form is to be used when a dog is adopted. Fill and sign both the contract form and the adoption checklist. At the time of adoption the adopter should sign and date the form to confirm that they have received the information and it gives the foster person a chance to note what may not have been done.
